Logansport Memorial Hospital announces no medical errors

The Indiana Department of Health issued the 2010 Indiana Medical Error Report on November 28, 2011. This is the fifth annual report and public release. Reporting requirements beginning in 2009 were different than in 2008 and earlier years. 

Logansport Memorial Hospital (LMH) has no reported medical events for 2010. In addition, LMH reported no medical events for 2009, 2008 and 2007. 

"Patient safety is a priority at Logansport Memorial Hospital," said David Ameen, President and CEO. "We have an extensive quality and safety program and serve as a member of the Indiana Hospital Association Council on Quality and Patient Safety." 

The state Council on Quality and Patient Safety and its regional coalitions are focused on several initiatives which have improved patient safety. Some of these initiatives include adoption og the surgical safety checklist, wristband standardization, implementation of "just culture" and ongoing patient safety training. 

Logansport Memorial Hospital follows the National Patient Safety Goals. Actions associated with the national goals to ensure patient safety include using at least two ways to identify (name and date of birth); medication safety; identification of patient safety risks; prevention of mistakes in surgery by marking the correct place on the patient's body where surgery is to be done and the use of time out during surgery. 

Infection prevention measures are taken at LMH. The use of hand cleaning guidelines have been adopted by all staff. LMH has been without Ventilator Associated Infections and Central Line Infections for 7 years, according to the Jeanette Huntoon, Chief Executive Nurse. "Another key factor that has led to our notable safety record is the fact that all clinical department are staffed by only certified and licensed staff," said Huntoon. 

Logansport Memorial Hospital has a Quality Council which is comprised of physicians, nurses, front line staff and management. They are responsible for overseeing the quality program at LMH. Patient safety is part of the LMH Quality Program.
