Hope Hospice of Rochester will now be open and providing services for patients at Logansport Memorial Hospital, as of Wednesday, February 29, 2012. The formulation of a partnership between these two organizations will allow for inpatient hospice services to help patients and their loved ones through the end of life in the privacy and comfort of individual rooms on the medical/surgical floor of the hospital.
“We are pleased to be open and located at Memorial Hospital so that patients needing hospice services can be cared for appropriately,” says Jon Mitchell, Vice President of Hope Hospice. “It is our commitment to the patients of this community to provide these services with compassion and contribute to the best quality of life possible for our patients during their time with us.”
Registered nurses with hospice training will see to the needs of both the patient and family. They will maintain the established plan of care for hospice services, with frequent communication with the patient's doctor and hospice team. Chaplain services are available 24 hours a day in the quiet setting of each room.
The Hope Hospice office is located in Medical Office Building East on the Lower Level, in Suite 25. The office is now state-certified and approved by CMS to be fully operational.
